Act as a Program Nurse educator for Patient Service

**主要工作職責

Major Duties Performed (Main Functions)**

  • 協助專案說明與個案收案並依照規範協助相關文件歸檔。

Assist in project explanations and enroll case, ensuring compliance with documentation standards.

  • 協助進行疾病與藥品衛教,回診前評估並回報醫療端。

Assist in health education regarding diseases and medications, conduct pre-visit assessments, and report findings to healthcare professionals.

  • 依規範進行不良事件通報。

Report adverse events according to established guidelines.

  • 個案管理、回診追蹤及資料整理。

Case management, follow up on appointments, and organize data effectively.

  • 依規範繳交專案服務報告。

Submit service reports in compliance with regulations.

  • 協助舉辦病友會 (依各專案需求)。

Assist in organizing patient association as needed.

學歷、經驗及專業知識要求

Education, Experience, & Specialized Knowledge Required

  • 醫藥相關科系,副學士學位或學士學位或同等學歷。

Medical and Health-Related Fields, Associate Degree or Bachelor's degree or equivalent.

  • 2年以上衛教師/個管師/臨床護理師經驗

2 years of experience as a Nurse Educator/Case Manager/Clinical Nurse.

  • 良好的人際交往/問題解決能力。

Good interpersonal and problem-solving skills.

  • 團隊合作精神,具備良好的溝通技巧,並以客戶服務為導向。

Team player with strong communication skills and a customer service-oriented approach.

  • 基本的電腦文書能力,包括 Word 和 Excel。

Basic PC literacy skills, including Word and Excel.

  • 具備衛教師相關經驗者最佳。

Experience As a Nurse Educator Is Preferred.
